  • Original Death Certificate issued by the local civil registrar or verified by the Philippine Statistics Authority (PSA)
  • Accident/Police Report for death due to accident
  • Affidavit of Undertaking executed by the claimant
  • Burial Permit
  • Official receipt of funeral expenses
  • Two (2) Valid IDs
