For individuals requesting an OFW Information Sheet from DMW, follow the steps below:

Step 1: Book an Appointment Online to Request a Certified Copy of the OFW Information Sheet

Launch your browser.
Type and search for OFW Information Sheet Appointment or click this link. Also, you may search for POEA official website or visit this link for Online Services.
Read the Warning advisory on using the online appointment system before proceeding.
Fill out the required fields, including:
OFW Details
Mode of Deployment
Year of records needed (records available starting 2001 only)
Mother’s Maiden Name
Purpose of Request
Review the details provided are true and correct.
Under the Select Appointment section, choose the nearest POEA Branch Office.
Choose an Appointment Date and Time.
Tick the box to agree to the Terms and Conditions of the OFW Records Online Appointment System Agreement.
Check I’m not a robot.
Again, double-check your details before clicking the Submit button.
After successful submission, the Appointment Form or Request for Verification/Certification of OFW Records will appear. To get a copy, click the Print button.

Step 2: Pick up OFW Information Sheet at DMW Office

Visit the POEA Branch Office you booked an appointment.
Present to the guard on duty your printed Appointment Form.
Next, submit the form and your requirements to the officer in charge and wait. You’ll get the certified copy of the OFW Information Sheet if you have complete requirements. But if incomplete requirements, you’ll get a verification status receipt indicating missing requirements.
Then, proceed to the releasing window to get the certified printed copy of the OFW Information Sheet. However, manual retrieval may be necessary if a Certificate of No Record is released. If this is the case, you’ll get a Verification Status Form with the pickup date of your record.
