- OFW Membership Record (MR) (from OWWA RWO)
- Copy of passport or Seaman’s Identification Record Book (SIRB) of the OFW member (from MARINA)
- One (1) copy of a 1×1 picture (from OFW Member)
- At least one (1) valid ID of the authorized claimant (from OFWs authorized claimant)
- Present an Authorization Letter with the ID of OFW if the claimant is not OFW (from OFWs authorized claimant)
- Accident Report/Masters Report for seafarer (from employer)
- Foreign Medical Certificate with English Translation (from OWWA-POLO)
- Medical Certificate with the indicated procedure of the medical examination/procedure of the doctor in the Philippines (from a competent medical practitioner from a legitimate medical institution/facility)
How to Apply OWWA Disability and Dismemberment Benefits
Follow the steps below if you wish to apply for OWWA Disability Benefits:
Step 1: Book an Appointment
- Book an appointment with the OWWA Regional Welfare Office (RWO), which covers the permanent address of the OFW member. You may book an appointment on the OWWA Mobile App or the OWWA Appointment Scheduler.
- Select the OWWA office where you want to have an appointment.
- Choose Social Benefits – Disability as the Type of Program and click the Next button.
- Select your preferred Appointment Date on the calendar. If there is no available slot, click the Next button on the upper right of the calendar to view the succeeding month.
- Then, pick the time for the appointment to proceed.
- Fill out the required fields in the Personal Information section. Also, provide a valid and active email to receive the appointment confirmation as proof.
- Double-check the entered details and confirm your appointment.
- Check your email for the appointment confirmation from OWWA. Print your appointment information and bring it to your visit.

- Go to the concerned OWWA RWO on the day of your appointment.
- Get a priority number (ticket) and wait for the OWWA representative to accommodate you.
- Show the ID or details of the OFW to the Membership Verification Desk.
- Next, obtain an application form and requirements checklist based on the membership status.
- Submit the documentary requirements and duly-accomplished claim form for verification.
- Once approved, show the claim stub and claim financial assistance thru cash or check payout.
